In-patients
Dietitians and Dietetic Support Workers have input into all the Community Hospitals across Leicestershire and Rutland. This will involve providing care for inpatients and arranging a care package for discharge.
Referral Form - where possible referrals should be via electronic systems available at the Community Hospital.
Outpatients
Outpatient clinics are held at a variety of local Community Hospitals and Health Centres.
These clinics are usually for people referred by Specialist Consultants or by the wider primary care team.
